How to Pay For Your Care at CommWell Health
At CommWell Health, we believe that every patient is entitled to quality affordable health care. CommWell Health accepts most insurance plans. CommWell Health also accepts self payments for those without insurance through its program, EagleCare. If you do not have insurance our Outreach and Enrollment staff can assist you with insurance applications or you can visit our Get Health Insurance (ACA) page for more information. We also accept Medicare and Medicaid.
CommWell Health offers Discounts based on Family Size and Income
What is the EagleCare Program?
CommWell Health discounts its services for anyone who qualifies based on income and family size. Typically the discount program benefits individuals who are uninsured, have high deductibles or have low income. This program provides a discount off of the health center’s normal charges and applies to all, of the health center’s services. If you think you may qualify, please review the following pages carefully as it provides all the information you need to get started.

How does the Eagle Care Program work?
We review your current income and family size to see if you qualify for the program.
What are the payment requirements?
We require your insurance co-payment at the time of visit. There are nominal fees required with the Eagle Care program regardless of the discount level. These payments are required at the time of registration for each visit. We accept cash, check, all major credit cards and debit cards.
Is CommWell Health a free clinic?
No, we are not a free clinic. There is a nominal fee for the health care services that we provide. However, because we are a Federally Qualified Health Center, we are able to offer reduced rates to people who qualify and no one will be turned away based solely on their ability to pay.

Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your health care will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for health care items and services.
- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services that are scheduled at least 3 days in advance. This includes costs of your visit with the provider and related costs like lab tests, prescription drugs, equipment, etc. that are associated with the visit.
- You can also ask your health care provider for a Good Faith Estimate before you schedule an item or service.
- You should receive a Good Faith Estimate inWriting at least 1 business day before your qualifying medical service or item or within 3 business days of your request for an estimate of costs prior to scheduling an item or service.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. To start the dispute process, contact CommWell Health Revenue Cycle Manager at 910-567-7065.
- Make sure to save a copy or picture of your Good Faith Estimate.
