The Eagle Foundation was founded in 2018 and would not exist without your generous philanthropic donations which help support the mission and vision of CommWell Health.

Any amount in support of the Foundation. Gifts are defaulted to the Eagle Foundation Campaign. Any donor whose lifetime gifts reach the Giving Levels will be recognized.
Make your dollars go further and make a commitment to one of the various pledge levels of the Eagle Foundation. You would be recognized as one of the Foundation’s top donors.
Are affordable and convenient! Choose what works best for you.. Simply designate an amount, frequency, and payment method.
Help us help your community! This fall, we have set a goal of $10,000 raised and 100 recurring monthly donations. You can help us reach this goal by setting up your recurring gift today. You can choose the amount that is comfortable for you.
CommWell Health’s school-based Community Mobile Dental Program known as KINSHIP (Kids In School Health Initiative and Partnership) provides dental care services during the school year at Johnston County Public Schools, Sampson County Public Schools and Clinton-City Schools. The demand for the current dental unit outweighs the annual capacity for one unit and we are seeking donations from businesses and organizations to go towards the operations of the mobile units.

The CommWell Health Eagle Foundation is offering a wonderful opportunity for you to support our health centers and patients. This shows your commitment to healthcare excellence through our Brick campaign, “Paving the Way…Brick by Brick”.
Sponsor a personalized brick to commemorate a loved one, promote your business, mark a special event in your life, or honor a family member, friend, or pet. The Brick fund will support the Eagle Legacy Garden. The Eagle Legacy Garden is a community beautification project that is located at CommWell Health’s Corporate Hub in Four oaks, NC.
Designate your gift to the Eagle Foundation for a specific program fund or unrestricted, meaning it will be used where the need is greatest at CommWell Health.
The Eagle Relief Fund provides financial assistance for CommWell Health colleagues who have suffered a substantial financial hardship related to an emergency. An emergency is defined as an unforeseen and unavoidable event of great magnitude which causes significant damage, destruction of personal property, or otherwise extraordinary personal catastrophe or crisis and results in financial burden.
The Smiles for Miles Dental Fund is a financial assistance program that helps individuals who are in need of dental care but can’t afford the expenses. The program is designed to help children 0-18 years of age who face financial difficulties and are uninsured or underinsured for dental work. Smiles for Miles Dental Fund relies on donations from individuals and corporations to provide financial assistance to those who need it most.
One of the great things about Smiles for Miles Dental Fund is that it not only provides financial assistance to individuals in need, but it also helps to promote good oral health and hygiene.
The mission of Giving Tuesday is parallel to that of the Eagle Foundation, which in summary is to inspire people to give; to improve the quality of life of others.
GivingTuesday is celebrated on the Tuesday following the U.S. Thanksgiving holiday. Join us this holiday season and give to the Eagle Foundation. Gifts of every size makes a difference and impact the lives of our patients and future generations.
The CommWell Health unrestricted fund is a valuable resource that allows the foundation to allocate funds where it is most needed. With no restrictions on how the funds can be used, the foundation can support a broad range of initiatives and programs.

About the Ceremony:
Started in 2023, the Christmas Tree Lighting Ceremony is a symbol of hope, unity, and joy for our communities. It is a gathering event that signals the start of the holiday season, a magical night of remembrance, camaraderie, hot cocoa/light refreshments, and the lighting of our stunning Christmas tree.

Tree Lighting Ceremony Donations:
A donation of $5.00 or more will add a light to the Four Oaks Operations Center Christmas Tree. The name(s) of your honoree will be read during the Tree Lighting Ceremony and added to the Program.
The CommWell Health Eagle Foundation allows our patients, colleagues, and community members to honor or celebrate a loved one as part of the Tree Lighting Ceremony. Designate a light on the Christmas tree in honor, in memory, or illuminate a life this holiday season.
The Christmas Tree Lighting Ceremony not only serves as a way to honor or celebrate someone, but also as a way for the CommWell Health Eagle Foundation to raise money for the Annual CommWell Health Eagle Foundation Campaign.
The annual campaign can be used towards the Eagle Relief Fund, Brick by Brick Fund, Smiles for Miles Fund, and numerous other funds and projects.
