CommWell Health Earns Joint Commission Accreditation and National Recognition from HRSA for Quality Excellence    

CommWell Receives Recognition for Exceptional Patient Care

Four Oaks, NC – August 19, 2025 – CommWell Health, a large not-for-profit Community Health Center in southeastern North Carolina, has earned The Joint Commission’s Gold Seal of Approval® for Ambulatory Health Care and Behavioral Health Care and Human Services Accreditation, plus certification as a Primary Care Medical Home (PCMH). It has also been recognized by the U.S. Department of Health and Human Services (HHS) Health Resources and Services Administration (HRSA) with five distinguished Community Health Quality Recognition (CHQR) badges for 2025.  

These awards highlight CommWell Health’s commitment to delivering exceptional patient care and improving community health outcomes. The Gold Seal is a symbol of quality that reflects a health care organization’s commitment to providing safe and quality patient care.   

CommWell Health underwent a rigorous, unannounced onsite review on May 20-23, 2025. During the visit, a team of Joint Commission reviewers evaluated compliance with Ambulatory Health Care, Behavioral Health Care and Human Services, and PCMH standards spanning several areas including provision of care, treatment and services, performance improvement, medication management, infection prevention and control, patients’ rights and responsibilities, emergency management, environment of care, and leadership.   

“As a healthcare accreditor, The Joint Commission works with healthcare organizations across care settings to inspire safer and higher quality of care that is more equitable and compassionate,” says Ken Grubbs, DNP, MBA, RN, executive vice president of Accreditation and Certification Operations and chief nursing officer, The Joint Commission. “Through collaborating on innovative solutions and evidence-based resources and tools, The Joint Commission helps drive improvement while maintaining accountability through our leading survey methods and standards. We commend CommWell Health for its commitment to advance safety, quality, equity and compassion for all patients.”   

The quality badges awarded to CommWell Health include five different CHQR badges in addition to Patient Centered Medical Home (PCMH) recognition. These badges are awarded annually to HRSA-funded health centers based on performance and recognize achievement in key areas such as preventive health, chronic disease management, technology adoption, and cost-effective care.
