CommWell Health Earns Seven HRSA 2026 Quality Recognition Badges for Exceptional Care

Four Oaks, NC – August 3, 2026 – CommWell Health (Formerly Tri-County Community Health Council, Inc.), a large not-for-profit Community Health Center, has been recognized with seven distinguished Community Health Quality Recognition (CHQR) badges by the U.S. Department of Health and Human Services (HHS) Health Resources and Services Administration (HRSA) in 2026. These awards highlight CommWell Health’s commitment to delivering exceptional patient care and improving community health outcomes in southeastern North Carolina. This recognition comes as CommWell Health marks its 50th golden anniversary this year, adding to a milestone year for the organization.

CommWell Health is proud to have earned the Health Center Quality Leader (HCQL) Gold badge, an honor awarded to health centers that are in the top 10% in the nation for Clinical Quality Measure (CQM) performance based on average Adjusted Quartile Rankings.

In addition to the HCQL Gold badge, CommWell Health received six additional CHQR badges which are awarded annually to community health centers based on performance and achievement in key areas such as access to care, clinical quality, patient health outcomes, and health information technology (HIT).

CommWell Health earned the following 2026 Community Health Quality Recognition badges:

  • Health Center Quality Leader Badge – Gold (Awarded to the top 10% of health centers nationwide)
  • National Quality Leader – Behavioral Health (Recognized top performance in depression screening, follow-up, and other behavioral health outcomes)
  • National Quality Leader – Heart Health (Awarded for achieving excellent targets based on the Million Heart Goals)
  • National Quality Leader – Diabetes Health (Recognized top performance in diabetes control and management)
  • High-Value Care (Honors health centers that successfully improve patient health outcomes while effectively managing patient cost growth)
  • Preventive Health (Exceeded national benchmarks in essential preventative screenings, including early childhood evaluations, depression screenings, and dental screenings)
  • Advancing Healthcare IT for Quality (Recognized achievements in Information Technology and to improve patient engagement)
