Four Oaks, NC – September 14, 2024 – CommWell Health, a not-for-profit, Community Health Center in southeastern North Carolina, was awarded 2024 Community Health Quality Recognition (CHQR) badges for quality achievements in enhancing access, reducing health disparities, addressing social risk factors, and advancing health information technology for quality.

The Community Health Quality Recognition (CHQR) awarded annually by the Health Resources & Services Administration, recognize and honor health centers like CommWell Health that have made notable achievements in improving health outcomes and providing high-quality care for patients in rural and underserved communities.

In addition to the CHQR and the Patient Centered Medical Home Recognition (PCMH) badges and recognition, CommWell Health is ranked for Patient Experience in the top 5% of the nation for behavioral health services and top 10% in the nation for medical and dental services through the Crossroads Group, Inc.
