Four Oaks, NC – CommWell Health, a not-for-profit, Federally Qualified Health Care (FQHC) system in southeastern North Carolina, recently earned four Community Health Quality Recognition (CHQR) awards by the Health Resources & Services Administration, a three-year Joint Commission Accreditation for Ambulatory Care and Behavioral Health Care as well as Primary Care Medical Home, and Patient Centered Medical Home Recognition from NCQA (National Committee for Quality Assurance).

The Community Health Quality Recognition (CHQR) awards recognize Health Center Program awardees like CommWell Health that have made notable achievements in the areas of access, health care quality, health equity, health information technology, and responding to COVID-19.
CommWell Health earned the following Community Health Quality Recognition badges:
- The Health Center Quality Leader (HCQL) recognition is awarded to Health Centers that demonstrate the best overall clinical quality measure (CQM) performance among all health centers.
- The Advancing Health Information Technology (HIT) for Quality recognition is awarded to Health Centers that use state-of-the-art technologies to deliver exceptional care.
- The Addressing Social Risk Factors award recognizes Health Centers that are screening for social risk factors impacting patient health, including poverty, race, and education.
- The Patient Centered Medical Home Recognition (PCMH) recognizes Health Centers that deliver high-quality, coordinated care that is focused on an individual patient’s needs.
CommWell Health underwent a rigorous, unannounced onsite review in July and has earned The Joint Commission’s Gold Seal of Approval® for Ambulatory Care, Behavioral Health Care, and Primary Care Medical Home Accreditation by demonstrating continuous compliance with its performance standards. “The Gold Seal is a symbol of continued excellence that reflects CommWell Health’s commitment to providing safe and quality patient care”, states Johnson Tilghman, Board Chair of CommWell Health.
The Patient Centered Medical Home Recognition from NCQA emphasizes the use of systematic, patient-centered, integrated care that supports access, communication, and patient involvement.
